Declaration, Authority, Consent, Waiver and Data Privacy Notice

Before submitting this application, please indicate your selection.

1. I declare that I am the duly authorized representative of the Supplier and possess full authority to accomplish, sign, submit, upload documents, provide information, make representations, execute declarations, and bind the Supplier in connection with this Supplier Registration and Accreditation.
2. I further represent and warrant that all information, documents, certifications, licenses, financial records, personal information, and other data submitted through this Vendor Accreditation and Procurement Portal are accurate, complete, authentic, and lawfully provided, and that I have obtained all necessary corporate approvals, authorities, and consents required for their disclosure.
3. I have read, understood, and voluntarily agree to the Consent, Authorization, Waiver, and Data Privacy Notice governing the collection, processing, storage, transmission, disclosure, verification, and lawful use of the information and documents submitted through this Portal.
4. I expressly authorize the Client identified through the validated Supplier Registration Code to collect, access, verify, process, retain, and use all submitted information and documents for supplier accreditation, procurement, due diligence, contracting, payment processing, audit, regulatory compliance, and other legitimate business purposes.
5. I likewise expressly authorize Integra Systems Corporation, as the exclusive owner, developer, operator, administrator, and technology service provider of this Vendor Accreditation and Procurement Portal, to collect, receive, host, store, process, back up, secure, maintain, transmit, and disclose the submitted information and documents solely to the extent necessary for operating the Portal, providing technical support, cybersecurity, system maintenance, disaster recovery, regulatory compliance, enforcement of contractual obligations, lawful governmental requests, and other legitimate purposes related to the operation of the Portal.
6. I acknowledge that Integra Systems Corporation is merely the technology platform provider and is not the procuring entity, buyer, or contracting party unless it is itself the identified Client. I further agree that the Client shall be responsible for its own processing and use of the submitted information after such information has been made available to it.
7. I voluntarily release and hold harmless Integra Systems Corporation, its directors, officers, shareholders, employees, affiliates, consultants, successors, assigns, licensors, and service providers from claims arising from the lawful processing, storage, transmission, disclosure, or use of the submitted information in accordance with Republic Act No. 10173, otherwise known as the Data Privacy Act of 2012, its Implementing Rules and Regulations, applicable laws, and the documented instructions of the Client, except where liability results from the fraud, willful misconduct, or gross negligence of Integra Systems Corporation as finally determined by a court of competent jurisdiction.
8. I understand that submitting false information, forged documents, or making unauthorized representations may constitute grounds for disqualification, revocation of accreditation, termination of business relations, and the institution of appropriate civil, criminal, or administrative proceedings.

I Do Not Agree I understand that failure or refusal to provide the foregoing declarations, authorizations, and consents prevents the processing of this Supplier Registration. Accordingly, I acknowledge that this application cannot proceed and will not be submitted for accreditation.
